How to choose a date:
1. Choose your date on the calendar by clicking the date.
2. From the drop down menu, click "Continue".
3. You will then be brought to the application form. Follow the instructions on the screen.

Please choose both dates requested within 3 calendar months.
How to choose multiple dates:
1. Choose your first date on the calendar by clicking the date.
2. Then, from the drop down menu that appears, choose "Add a time".
3. Then, choose your second date.
4. From the drop down menu, choose "Continue"
5. You will then be brought to the application form. Follow the instructions on the screen.

Please choose all three dates requested within 3 calendar months.
